There are several factors to consider when choosing the right nursing home for you or your loved one.

Is the facility near friends and family? Is it clean? What are the perks? These are just some of the several questions that typically surface when researching long-term care facilities.

The U.S. Centers for Medicare and Medicaid Services ranked 46 facilities within a 25-mile radius of Sacramento based on health inspections, staffing and quality measures. The highest-rated centers near the capital city are in Sacramento, Carmichael, Elk Grove, Folsom, Davis and Woodland.

The list, which was last updated Jan. 25, includes more than a dozen facilities flagged for resident harm or potential neglect.

Here’s a checklist, provided by the National Institute on Aging, on how to choose a nursing home:

  • Make a list of potential nursing homes

  • Decide which amenities are important to you

  • Meet both the facility and nursing directors and ask questions

  • Visit the facility again without calling ahead

  • Get a second opinion of the nursing home from a healthcare provider

  • Carefully read the contract
